About the Role Horizon Healthcare Staffing is seeking an experienced MDS / Care Plan Coordinator (RN) for a temp-to-perm opportunity with one of our long-term care partners. This role is essential in ensuring accurate resident assessments, care planning, and regulatory compliance.

Key Responsibilities

• Complete Minimum Data Set (MDS) assessments and comprehensive care plans for all residents
• Perform initial, quarterly, annual, and significant change assessments
• Develop and update interdisciplinary care plans and summaries
• Coordinate and attend IDT (Interdisciplinary Team) meetings
• Ensure compliance with state and federal regulations
• Maintain accurate documentation for Medicare/Medicaid and billing purposes
• Collaborate with nursing staff and department heads on care plan implementation
• Conduct chart audits to ensure documentation accuracy and completeness
• Communicate with residents and families regarding care planning

Additional Responsibilities

• Assist with admissions and discharges
• Support quality assurance initiatives
• Stay current on PDPM, CMS, and regulatory changes
• Participate in facility trainings and meetings as needed

Qualifications

• Active RN license (state-specific)
• 2+ years of clinical experience in long-term care or skilled nursing
• Prior MDS experience required
• Familiarity with PointClickCare or similar EMR systems
• Strong organizational and communication skills
• AANAC certification preferred
