Position Overview
Horizon Healthcare Staffing is seeking a New York Licensed Registered Nurse (RN) to serve as a Nursing Home Surveyor in Syracuse, NY.
In this role, you will conduct onsite regulatory surveys and investigations of nursing homes, Transitional Care Units (TCUs), and Adult Day Health Care Programs (ADHCP) in accordance with New York State Department of Health (DOH) regulations.
This is a field-based, travel-heavy position ideal for RNs with regulatory, investigative, utilization review, or long-term care experience.
Responsibilities
Responsibilities:
- Conduct onsite surveys of Skilled Nursing Facilities (SNFs), long-term care facilities, TCUs, and ADHCPs
- Interview facility staff, residents, and family members
- Investigate allegations of abuse and neglect
- Perform surveillance and monitoring activities to determine regulatory compliance
- Participate in emergency surveys and Immediate Jeopardy monitoring situations
- Review clinical documentation and facility records
- Prepare detailed investigative reports
- Draft and submit Statements of Deficiencies (SOD) within required timeframes using Principles of Documentation
- Collaborate as part of an interdisciplinary survey/investigation team
Qualifications

Required:
- Active New York State Registered Nurse (RN) license
- Valid driver’s license and ability to travel to onsite assignments
- Ability to obtain Surveyor Minimum Qualifications Test (SMQT) certification within 12 months of employment
- Strong computer proficiency, including Microsoft Office and ability to learn new regulatory systems
Experience (One of the Following Required):
- Two (2) years of experience in utilization review, claims adjudication, medical review, fraud investigation, surveillance, or monitoring activities; OR
- Three (3) years of clinical or administrative healthcare experience; OR
- Bachelor’s degree in Nursing and two (2) years of clinical or administrative experience
