Horizon Healthcare Staffing is searching for an Oncology Nurse Navigator for one of our clients located in Hackensack, New Jersey. The candidate will be asked to work within the RN scope of practice in the State(s) assigned to perform telephonic assessments, identifying and anticipating patients’ needs, coordinating care based on telephonic pathways to decrease avoidable visits to the Emergency Rooms (ER) and unplanned admissions and readmissions.

• Supports Medical and Surgical Oncology, as well as Radiation Oncology.
• Provides supportive care to patients that require acute care in the assigned practices.
• Monitors patients receiving oral chemotherapy for adherence and toxicity management.
• Initiates, completes and updates a care plan that strongly encourages patient, family and caregiver engagement and when appropriate begin Advance Care Planning conversation, assist in obtaining Provider or Medical Life sustaining Treatment (POLST/MOLST) and early referrals to outpatient palliative care and hospice programs and facilitate survivorship planning.
• Works with multi-disciplinary teams that care for the patient and consult with physicians and other advance practice clinicians when appropriate.
• Adheres to the policies and procedures of the client.
• Promotes professional growth and enhances the quality of service and reputation of the practices assigned.

Job Duties and Responsibilities:

• Triage patients phone calls, ensuring quality patient care and adherence to standardized nursing procedures and evidence-based pathways and quality requirements provided
• Schedules patients appropriately based on needs and triage pathways
• Returns patients calls based on acuity and urgency of need as well as follows-up on patients previously called according to triage pathways and care management algorithms
• Ensure that the assigned patients have a documented care plan that meets the 13 components in the Institute of Medicine (IOM) Care Management Plan
• Provide telephonic care transitions services such as timely follow-ups post hospitalizations, ED visits, and stays in other institutional settings like Skilled Nursing Facilities and coordinate Home Health services for patients when appropriate
• Provide education, support and other patient navigation requirements required by both public and private payers to patients, families and significant others through telephonic and in person interaction
• Manage patient referrals by identifying all providers caring for the patients; resolve duplications if any and when appropriate refer patients with behavioral health needs to social workers and other licensed mental health professionals and/or community agencies
• Performs required documentation in the Triage or EMR platforms
• Actively participates in data driven performance activities required by public and private payer programs

Knowledge and Experience:

• Current RN license in New Jersey
• 3+ years’ experience in clinical oncology
• Care management experience in inpatient, outpatient or health plan settings preferred
• Demonstrated evidence of leadership qualities, excellent oral and written communications and counseling skills
• Proficiency in MS Office applications (Outlook, Word, Excel, Power Point) as well as E-mail and smart phone technology
• Experience in documenting in Electronic Health/Medical Records (EHR/EMR)
• Local knowledge of community services and resources preferred
• BCLS certification required and ACLS certification a plus
• Knowledge of QOPI and ASCO standards a plus

Education:

• Bachelors of Science in Nursing or working towards degree and must complete within 2 years of employment
• Certifications in Oncology, care management, Quality and medical home a plus
• Master’s degree a plus

Work Environment:

• Works in medical facility but will need to occasionally be in business setting
